Northpoint Recovery Holdings, LLC, launched the Northpoint platform in 2015 and now operates 17 behavioral health facilities across the Western U.S. The company provides evidence‑based treatment for adults with substance use and co‑occurring disorders and offers mental health treatment for adolescents through the Imagine by Northpoint brand.
Position Summary
The LPN – PRN will provide and oversee medical services for a caseload of patients, coordinating treatment plans and discharge plans with the multidisciplinary team. Working under the supervision of an RN or provider, the LPN will perform duties in line with patient care protocols, document all responsibilities, and communicate regularly with staff, patients, families, and outside agencies.
Essential Responsibilities and Duties
- Interact routinely with patients, observe behaviors, and communicate significant observations to the multidisciplinary team.
- Administer medications and engage in the education process.
- Use de‑escalation skills when exposed to volatile patients to maintain personal safety.
- Provide admission, orientation, education, and discharge plans to patients and families.
- Consult with the treatment team to develop and revise treatment plans.
- Participate in maintaining a clean, orderly, and safe environment.
- Handle admissions and discharges of patients.
- Obtain vital signs, monitor symptoms, and document in the patient record.
- Comply with all laws, regulations, and scope of licensure.
- Monitor patients continuously to assess ongoing needs and facilitate medical treatment.
- Document medical assessments, diagnoses, progress notes, and other planning items promptly.
- Actively participate as a member of the multidisciplinary team and maintain effective relationships.
- Maintain professional boundaries with patients.
- Oversee and participate in quality assurance and compliance activities.
- Provide medical updates to utilization review and managed care representatives.
- Adhere to company policies and procedures.
- Maintain confidentiality in accordance with established policies.
- Perform other clinically appropriate services and special projects as assigned.
Qualifications and Requirements
- Current, unencumbered, active LPN license in the state of employment.
- Minimum age 21 years.
- Current CPR/First Aid/AED certification.
- Knowledge of electronic medical record systems (KIPU preferred).
- Enthusiasm, passion for working with people, and a drive to improve patients’ lives.
- Maintain applicable state licensure throughout employment.
- Ability to work with a diverse population and possess interpersonal skills required for patient care.
- Demonstrated ability to maintain personal boundaries and build rapport with patients.
- Understanding and adherence to ethical standards of the licensure board.
