Job summary

Cordova Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide compassionate resident care in a PRN, flexible schedule setting. You will work with RNs, CNAs, and the care team to administer medications, monitor residents, and support individualized care plans.

The role emphasizes accurate documentation, adherence to infection control, and collaboration with the interdisciplinary team to ensure high-quality outcomes.

Qualifications

• Current and valid LPN license.
• Strong clinical and medication administration skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a team.
• Compassionate, resident-focused approach to care.
• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as ordered.
• Monitor, document, and report changes in residents’ conditions.
• Provide direct nursing care and assist residents as needed.
• Collaborate with the interdisciplinary team to support individualized plans of care.
• Maintain accurate and timely clinical documentation.
• Provide education and support to residents and their families.
• Follow infection prevention, safety, and regulatory standards.
