Job summary

Hartford HealthCare Medical Group is seeking a Licensed Practical Nurse to collaborate with the care team and provide direct patient care at our Bridgeport location. This role supports established care plans and documentation under RN/MD supervision.

Responsibilities include performing therapeutic measures and collecting patient data to meet goals. Bilingual English/Spanish is preferred to assist diverse patient needs.

Qualifications

Job Description

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or MD, to assist in achieving established patient care outcomes

Qualifications

• Current Connecticut Licensed Practical Nurse license required. Medical/Surgical office experience preferred.
• Bilingual- English Spanish preferred
