Center at Park West in Pueblo, Colorado seeks a dedicated LPN to join our nursing team. You will provide high-quality resident care under physicians and Medical Director, including medication administration, wound care, and vital sign monitoring.

You will collaborate with physicians and staff, document care in electronic health records, and participate in quality improvement initiatives to maintain a safe, therapeutic environment.

Qualifications

• Current, unencumbered LPN license.
• Ability to make independent decisions and exercise sound judgment.
• Strong communication and interpersonal skills.
• Knowledge of nursing and long-term care regulations.
• Ability to work effectively in a team environment and supervise other personnel as needed.
• Patience, compassion, and a positive attitude.

Responsabilités

• Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
• Observe, assess, and report changes in resident condition to physicians and the nursing team.
• Assist in the development and modification of resident care plans.
• Document resident care accurately and thoroughly in electronic health records.
• Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
• Participate in facility surveys and quality improvement initiatives.
• Maintain a safe and therapeutic environment for residents.
