Job summary

Blue Ridge Care is seeking an LPN to join our hospice team. You will provide direct patient care under the supervision of a Registered Nurse, following individualized care plans to ensure comfort and support.

The role collaborates with an interdisciplinary team to meet physical, emotional, and spiritual needs. We offer full-time, part-time, or PRN positions with competitive pay, shift differentials, and a comprehensive benefits package.

Qualifications

• Graduate of an accredited practical nursing program.
• Current Virginia LPN license.
• CPR certification required.
• At least two years of nursing experience (hospice or community health preferred).

Responsibilities

• Provide direct patient care under the supervision of an RN.
• Implement nursing interventions per care plan.
• Document care accurately in the EMR at the bedside.
• Educate patients and families to promote comfort and understanding.
• Manage equipment and medical supplies efficiently.
• Provide pain and symptom management and report changes to the RN.

About the Role

We’re seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team—available full-time, part-time, or PRN. As an LPN, you’ll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You’ll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.

• Provide direct patient care as outlined in the plan of care established by the Registered Nurse.

• Implement nursing interventions based on assessments and care plans.

• Document care accurately and timely in the EMR—right at the bedside.

• Educate and support patients, families, and caregivers to promote comfort and understanding.

• Manage equipment and medical supplies efficiently.

• Provide appropriate pain and symptom management, and communicate changes to the RN or provider.

• Participate in team meetings, interdisciplinary discussions, and ongoing professional development.

What You’ll Bring

• Current LPN license in Virginia.

• CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred).
