Rochester Regional Health is seeking an experienced Licensed Practical Nurse at DeMay Living Center in Newark, NY. The LPN will deliver professional care to long-term and post-acute residents during night shifts and supervise CNAs to ensure high-quality patient outcomes.

Responsibilities include accurate medication administration, comprehensive documentation, and timely communication with physicians. A strong clinical skill set and leadership abilities are essential for success in this role.

Qualificações

• Graduate from an accredited School of Practical Nursing.
• Current licensure in the State of New York as an LPN.
• BLS certification required by AHA.

Responsabilidades

You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.Key Responsibilities:Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.Performs treatments and documents progress of same as per physician orders and facility protocol.Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.Directs CNA assigned to the team in the timely completion of quality individualized personal care in compliance with resident's individual care plans. Ensures CNA documentation is accurate and complete.Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.Contacts physicians, transcribes orders and follows up with appropriate action to institute orders in timely manner.Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.Completes pre-reviews of medication administration records as per facility protocols.Completes required MDS documentation and all related observations in a timely manner.Supports CNA team through assisting in direct ADL care of residents.Minimum Qualifications:Graduate from an accredited School of Practical Nursing.1 year experience preferred.Required Licensure / Certification Skills:Current licensure in the State of New York.EDUCATION:LICENSES / CERTIFICATIONS:BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - NYS Office of ProfessionsPHYSICAL REQUIREMENTS:Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation.
