Live Well Homecare Agency, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct patient care across long-term care, skilled nursing, and post-acute facilities. You will administer medications, monitor patient conditions, and support ongoing care plans within an interdisciplinary team.

You will document observations in the EHR, communicate changes to the care team, and uphold compassionate bedside manner and teamwork.

Formación

• Active LPN license.
• Proficient medication administration and patient monitoring.
• Accurate documentation in electronic health records.
