Licensed Practical Nurse / LPN – Hospice – PRN
The Licensed Practical Nurse / LPN assists in planning, coordinating, and caring for hospice patients in accordance with the patient’s plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with interdisciplinary team members to help achieve positive care outcomes. This is a PRN/on‑call position requiring approximately 40‑50 hours per month (5 days) of availability.
Essential Responsibilities
- Provide direct patient care as defined in the State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse.
- Perform focused/limited nursing visits, including telephonic triage and night/weekend coverage.
- Participate in backup rotation with the nursing team when the primary nurse is off.
- Carry out other duties assigned by Hospice Leadership.
- Collaborate with hospice team members regarding patient care.
- Perform focused/limited nursing assessment, recognize signs and symptoms of end‑stage medical conditions, and implement planned interventions as delegated by a registered nurse/clinician.
- Gather and review documentation in the electronic medical records, ensuring accuracy, completeness, and compliance with regulations and certification standards.
- Administer medications and treatments as prescribed by the patient’s clinician.
- Evaluate patient response to treatments/medications and notify hospice providers and/or registered nurses of concerns.
- Contribute to the development, implementation, or modification of the individualized patient’s plan of care under the direction of the registered nurse/clinician.
- Provide health teaching/education and support to patients and caregivers related to end‑of‑life care needs.
- Assist with hospice chart auditing as requested.
Experience and/or Education Required
- Graduate of an accredited LPN training program.
- Minimum one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facility, memory care unit, or acute care setting (preferably medical/surgical).
- Current unencumbered LPN license in the State of Oregon.
- CPR certification at hire or within six months.
- Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
Knowledge, Skills and Abilities
- Knowledge of federal, state and local regulations, Medicare Conditions of Participation, and HIPAA privacy rules.
- Demonstrate nursing abilities and counseling in a hospice setting.
- Practice within the scope of license, certification or registration in accordance with law and regulation.
- General computer skills and working knowledge of Electronic Medical Records systems, MS Office, Outlook, and the internet.
- Excellent interpersonal skills and the ability to tolerate and manage stress and grief.
- Work independently and collaboratively, and meet deadlines under pressure.
- Initiate and utilize innovative techniques and ingenuity.
- Interact with various departments throughout the organization.
- Work flexible hours, including on‑call or after‑hours duties.
- Plan, prioritize and coordinate multiple projects or tasks.
- Adhere to organizational standards, policies and procedures.

- Communicate clearly for at least six hours per day.
- Perform physical tasks such as standing, walking, sitting, repetitive finger and wrist movements, lifting, carrying, and operating a motorized vehicle in all weather conditions for several hours.
Working Conditions
- Work environment: Indoor/Office, Community, Outdoor Exposure.
- Member/Patient facing: Telephonic, In Person.
- Hazards may include physical, ergonomic, and biological hazards.
- Equipment: General office equipment and/or mobile technology.
- Travel: Requires travel outside the workplace at least weekly; personal vehicle may be used.
- Schedule: This position may occasionally require more than eight hours per day and may be assigned on‑call or after‑hours duties.
- Background checks, immunization review, and drug screening are required for employment.
