Job summary

St. Luke’s Hospital is seeking an LPN to perform primary nursing functions for homecare residents, evaluating care in line with Nursing Standards and Hospital Policies.

You will conduct start-of-care, recertifications, resumption-of-care and discharge OASIS assessments in accordance with HIPAA guidelines. The ideal candidate has an accredited LPN program diploma, Missouri LPN licensure, and at least one year of general nursing experience in a homecare setting.

Qualifications

• Graduate of accredited Licensed Practical Nurse program.
• Current licensure as a Practical Nurse in Missouri.
• One year of general nursing practice.

Responsibilities

• Evaluate designated group of residents in accordance with Nursing Standards and Hospital Policies.
• Provide quality nursing care to patients in their residence as prescribed by physician and HIPAA policies.
• Perform start-of-care, recertifications, resumption-of-care and discharge Comprehensive OASIS assessments.

Position Summary:

The LPN performs the primary functions of a registered professional in evaluating a designated group of residents in accordance with all established Nursing Standards and Hospital Policies. Provides quality nursing care for patients in their place of residence, as prescribed by the physician and in compliance with the standards of the Homecare Agency Infection Control Policies and HIPAA Policies. Performs and completes start-of-care, recertification’s, resumption-of-care, and discharge Comprehensive OASIS assessment. Performs all duties in a manner consistent with St. Luke’s mission and values.

Education, Experience, & Licensing Requirements:

Education: Graduate of accredited Licensed Practical Nurse Program

Licensure: Current licensure as a Practical Nurse in the state of Missouri

Experience: One (1) year of general nursing practice
