Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts.
• Current BLS/CPR certification.
• Current TB test or chest X-ray.
• Strong clinical skills and attention to detail.
• Compassionate, patient-focused approach to care.
• One year of nursing experience preferred (new graduates encouraged to apply).

Responsibilities

• Deliver skilled nursing care in accordance with physician orders and care plans.
• Assess and monitor patient conditions, recognizing changes in physical or emotional status.
• Collect, analyze, and document patient data to support care planning and coordination.
• Assist in developing and implementing individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and families to support safety, independence, and overall well-being.
