Actify Home Care, Inc. seeks an LPN to deliver in-home patient care across our South Broward County territory. You will follow RN-directed treatment plans, document patient progress, and coordinate care with the supervising RN in a fast-paced clinician-led environment.

Qualifications

• Active Florida Licensed Practical Nurse (LPN) license required.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Kinnser/WellSky experience is a major plus.

Responsibilities

• Provide high-quality, one-on-one care to geriatric patients in patients' homes.
• Implement and follow treatment plans established by supervising RN.
• Chart patient progress and treatment responses accurately and efficiently.
• Coordinate and communicate patient status regularly with the supervising RN.

LPN Compensation (1099):

• Hi-Tech / Complex Care Visits: $35.00
• Routine Follow-up Visits: $30.00
  

Territory: South Broward County: Hollywood, Pembroke Pines, Miramar, Davie, Weston, Hallandale Beach, and surrounding areas.

What You’ll Do as an LPN:

What You Need:

• Transportation: Valid Driver's License and reliable transportation.
