Job summary

Professional Case Management in Tennessee is seeking LPNs/LVNs to provide in-home healthcare services to patients, including veterans. You will develop one-on-one relationships, monitor health, and support quality of life through compassionate care.

You have the freedom to create your own schedule, enabling time for school, travel, volunteering, or family activities alongside meaningful clinical work. The position offers competitive hourly pay and a comprehensive benefits package.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted LPN/LVN license in the state(s) of practice.
• Prefer minimum of one year experience as an LPN/LVN in an acute care setting or equivalent; will consider new graduates.
• Current CPR certification and TB test.
