Phoenix Home Care and Hospice is hiring a Licensed Practical Nurse (LPN) for home health in Springfield, MO and surrounding communities. Earn between $60,000 and $75,000 annually with a $2,500 sign-on bonus.

Qualifications

• Current Missouri or Compact LPN license.

Responsibilities

• Deliver skilled nursing care in patients' homes per physician orders and care plans.
• Perform comprehensive patient assessments and monitor condition changes.
• Educate patients and families to promote confidence and independence at home.
• Collaborate with RNs, therapists, physicians and the care team.
• Maintain accurate documentation using AI-supported charting technology.
• Participate in quality improvement and ongoing professional development.

What You'll Do

• Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans
• Perform comprehensive patient assessments and monitor changes in condition
• Educate patients and families to promote confidence and independence at home
• Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team
• Maintain accurate documentation using our AI-supported charting technology
• Participate in quality improvement initiatives and ongoing professional development

Schedule

Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.

What We're Looking For

• Current Missouri or Compact LPN license
