AccentCare, Inc. is hiring a Licensed Practical Nurse (LPN) for home health in the Maplewood area. Coverage includes Oakdale and surrounding areas with Monday–Friday, 8:00 a.m.–5:00 p.m.

work hours. Compensation is presented as an annual estimate of $66,000 to $77,000, paid on a per-point basis. The LVN/LPN will provide direct patient care under RN supervision, documenting progress and changes for pediatric to geriatric patients.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently licensed LPN/LVN in the state of operation.
• One year of LVN/LPN work experience preferred.
• Ability to provide nursing visits and maintain medical records.
• CPR certification required.
• Licensed driver with ability to travel to all locations.

Responsibilities

• Perform technical nursing functions and direct patient care per plan of care under RN supervision.
• Promote and maintain optimal client care under RN supervision.
• Maintain a safe patient environment.
• Identify abnormalities in patient data and report to case manager.
• Administer prescribed medications as allowed by law and assess potential drug interactions.
• Re-evaluate client nursing needs and report findings to Case Manager.
• Counsel and instruct clients and families in nursing needs.
• Prepare clinical and progress notes and submit documentation timely.

What You Need to Know

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult and geriatric patients.

If you meet these qualifications, we would love to meet you:

• Graduate of an accredited practical or vocational nursing program
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
• One year of LVN/LPN work experience
• Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations

Responsibilities

• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members.
• Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician.
• Re-evaluates client's nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to client
