Job summary

Enhabit Home Health & Hospice in Connecticut seeks a licensed nurse to provide skilled nursing care in patients' homes. You will educate families, follow physician orders, and implement the plan of care with compassion and professionalism.

Qualified candidates are graduates of an approved nursing program, hold an active CT license, have at least one year of nursing experience, and possess a valid driver's license with reliable transportation to serve patients across the region.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed nurse.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled nursing services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Responsibilities

Qualifications

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president for operations or regional director.
• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
