Job summary

BAYADA Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our Pitt County Visits office on a part-time basis. You will provide in-home health services to adult clients across Pitt and Craven counties, with a requirement of at least one year of LPN experience.

You will administer medications, perform wound care, and monitor client conditions while documenting actions and changes.

Qualifications

• Current North Carolina LPN license (active).
• Minimum of one year of recent, verifiable LPN experience.
• Graduate from an accredited nursing program (diploma/degree).
• Ability to travel to cases as assigned.
• Strong interpersonal and independent-work skills.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, provide wound care, and perform treatments.
• Monitor client conditions and report changes to Clinical Manager.
• Document doctors' orders and client observations accurately.
• Coordinate case management and use EMR on a tablet.

BAYADA Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team part time in our Pitt County Visits office .

This position will be providing home health services to our adult clients on a per visit basis in homes throughout Pitt and Craven counties.

One year prior clinical experience as a licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Responsibilities

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN)

• A current license as a Licensed Practice Nurse in the state of North Carolina .
• A minimum of one year of recent, verifiable experience as an LPN.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information

Base Pay, depending upon qualifications: $35-$40 per point
