Deliver direct patient care and assist with daily living activities.
Observe and monitor resident health and vital signs, documenting accurately.
Consult with Registered Nurses (RN) and Unit Managers regarding care plans.
Support resident admissions, discharges, and transitions of care.
Maintain compliance with facility policies, procedures, and state regulations.
Current Licensed Practical Nurse (LPN) license in Massachusetts.
Long‑term care experience preferred but not required.
Strong interpersonal skills and a compassionate, patient‑focused demeanor.
Ability to work collaboratively with the care team.
