Job summary

Discovery Senior Living in Ellicott City, MD, is seeking a Licensed Practical Nurse (LPN) for Heartlands Senior Living. You will provide compassionate nursing care in accordance with care plans to support resident safety, dignity and well-being.

Requirements include a current LPN license (RN acceptable in some states) and at least 2 years of licensed nursing experience. The role requires flexibility for nights/weekends and proficiency with electronic records.

Qualifications

Position Highlights

• Full Time – Day Shift 7am–3:30pm

Responsibilities

• Provide nursing care to residents in accordance with established care plans, policies, and procedures.
• Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
• Support health promotion, disease prevention, and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning, service plan updates, and resident evaluations as required.
• Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high‑quality care delivery.
• Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
• Support training and guidance for care staff as appropriate.
• Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
• Participate in quality assurance initiatives, audits, and compliance activities as needed.
• Perform other duties as assigned.

Qualifications

• Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations.
• Minimum of two (2) years of Licensed Nurse experience required.
• Experience in assisted living, long‑term care, or a skilled nursing environment preferred.
• Strong clinical judgment, problem‑solving, and decision‑making skills.
• Effective organizational, time‑management, and communication skills.
• Ability to manage multiple priorities and delegate appropriately.
• Demonstrated commitment to seniors and person‑centered care.
