Job summary

Altru Health System's Devils Lake clinic is seeking a Licensed Practical Nurse (LPN) to support patient assessments, health history documentation, and admission/discharge processes.

You will collaborate with a multidisciplinary team to provide compassionate patient care, education, and timely nursing interventions, while monitoring progress and adjusting plans as needed.

Qualifications

• Active LPN license in North Dakota or a compact state.
• BLS certification required within 90 days of start date.

Responsibilities

• Perform and document assessments appropriate to licensure.
• Assist in health history, admission processes, and discharge planning.
• Provide patient care and education as needed.
• Communicate patient progress to the team and adjust workload accordingly.
• Collaborate with multidisciplinary team to implement an integrated care plan.
• Identify early signs of changes in patient condition and report promptly.

Location: Altru Clinic | Devils Lake, 1001 7th St. NE Devils Lake, ND 58301. Pay Range: $20.75 - $31.12.

Summary

The Licensed Practical Nurse (LPN) performs and documents assessments appropriate to licensure. This position ensures health history and admission process is completed and that interventions or referrals are made. The LPN identifies discharge planning and education needs, provides patient care, and performs personal cares. The Licensed Practical Nurse communicates patient progress, and adjusts own workload.

Essential Job Functions

• Assists the registered nurse or other licensed practitioner in identifying patient needs, priorities of care, and goals based on patient's age, ethnic origin, economic status, lifestyle, creed, or philosophical beliefs.
• Contributes to evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
• Modifies the plan of care in collaboration with licensed practitioner.
• Demonstrates competency and ability to carry out department specific functions in accordance with department's standards of care, policies, and procedures.
• Performs other duties as assigned to meet the needs of the department/organization.
• Functions as a member of the multidisciplinary team, contributing to implementation of an integrated patient-centered care plan.
• Assigns or delegates responsibility for performance of nursing interventions to other persons who have necessary skills and competence to accomplish safely.
• Identifies early warning signs of a change in a patient's condition and reports appropriately.
• Implements nursing interventions and prescribed medical regimens in a timely and safe manner and documents nursing care provided accurately and timely.
• Collaborates and communicates relevant and timely patient information with patients/families and other multidisciplinary team members to ensure quality and continuity of care.
• Reports patient's self-reported symptoms of pain promptly according to standard operating procedure.
• Participates in health education approved by a licensed practitioner.
• Assists in discharge planning, appropriate education and counseling based on identified needs to optimize patient/family understanding.
• Performs other duties as assigned or needed to meet the needs of the department/organization.

Certification and Licensure

• Licensed Practical Nurse (LPN) – North Dakota Board of Nursing (NDBON) or Compact State Agreement.
• Basic Life Support (BLS) – American Heart Association (AHA) required within 90 days of start date.

Language Requirements

This position requires proficiency in reading, writing, and speaking English to ensure effective communication in the workplace and with patients, families, and team members.

Physical Demands

• Occupation requires sitting occasionally, standing frequently, walking frequently, stooping/bending frequently, reaching, twisting occasionally, handling/fingers continuously, seeing continuously, hearing continuously.
• Weight Demands: Lift floor to waist level, carry, push/pull, slide/transfer – medium (20–40 pounds).
