Job summary

Oak Knoll Health and Rehabilitation LLC is seeking a skilled Licensed Practical Nurse (LPN) for the evening shift. You will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs and physicians to ensure high-quality care in a long-term care setting.

Key duties include medication administration, vital sign monitoring, ADLs assistance, and maintaining thorough documentation.

Qualifications

• Current and valid LPN license.
• Strong clinical skills with nursing care.
• BLS/CPR certification.
• Excellent communication and teamwork.
• Experience in long-term care preferred.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.
• Collaborate with the rehabilitation team to support residents' recovery goals.
• Maintain accurate, timely documentation per facility policies and regulations.
• Provide emotional support and education to residents and families.
• Ensure a safe, clean environment adhering to infection control protocols.

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and interdisciplinary team members to ensure the highest standards of care in our long-term care and rehabilitation setting.

Qualifications:

1. Current and valid LPN license
2. Strong clinical skills, including medication administration and wound care.
3. Excellent communication and interpersonal skills.
4. Ability to work effectively in a team-oriented environment.
5. Compassionate, patient-focused approach to care.
6. BLS/CPR certification required.
