The Licensed Practical Nurse (LPN) at Rochester Regional Health ElderONE – Seneca Towers in Rochester, NY provides direct patient care under RN supervision within a PACE-based ElderONE program. Schedule includes Day or Evening shifts, 40 hours per week, with every other weekend, and occasional travel to affiliated locations.

Responsibilities include observing patient status, assisting with treatment plans, delivering routine nursing care, documenting care, and attending meetings.

Formación

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• BLS certification from the American Heart Association (AHA).

Hours Per Week: 40 hours, Full-Time

Schedule: Day OR Evening Shifts: (Days 8am-4:30pm, Evenings 2pm-10:30pm), every other weekend required.

This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

SUMMARY:

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

REQUIRED LICENSURE / CERTIFICATION SKILLS

• Graduate from an accredited School of Practical Nursing.

• Current LPN licensure in State of New York.

• Basic Life Support (BLS) Certification from the American Heart Association (AHA).

PREFERRED QUALIFICATIONS

• 1 year experience

• 1 year of experience working with a frail or elderly population preferred.

PHYSICAL REQUIREMENTS:

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision‑making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask‑fit testing, and physical exams are common requirements.
