Job summary

Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) for the ElderONE- Homecare program in Rochester, NY. This full-time role entails direct patient care under the supervision of an RN, with a 40-hour workweek and a Monday–Friday schedule, occasional weekends, and travel to multiple locations.

The position requires NY LPN licensure, BLS certification, and at least 1 year of elderly care experience. Mileage reimbursement is provided for travel between sites.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).

Responsibilities

• Conducts and documents thorough observation of each patient’s medical status on admission and throughout treatment.
• Assists in implementation of the patient’s treatment plan under RN direction.
• Provides routine nursing services as directed by the RN.
• Complies with evaluation, treatment, and documentation guidelines of the department.
• Completes required documentation accurately and timely.
• Provides regular patient status updates to appropriate personnel.
• Attends and participates in department/facility meetings as required.
• Maintains compliance with hospital, regulatory, and professional standards and policies.
• Pursues continuing education opportunities to maintain clinical competency.
• Performs other duties as assigned.

Job Title: Licensed Practical Nurse (LPN), ElderONE(Full Time, Days)

Department: ElderONE- Homecare

Location: 2066 Hudson Avenue, Rochester, NY 14617 (community of the Hudson and North Park Campuses)

Hours Per Week: 40

Schedule: Monday - Friday, 8:30am to 4:30pm with occasional weekends

This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

SUMMARY

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

RESPONSIBILITIES

• Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.

REQUIRED LICENSURE / CERTIFICATION SKILLS

PREFERRED QUALIFICATIONS

• Strong communication, customer service, and interpersonal skills highly desired.
• 1 year of experience working with a frail or elderly population preferred.
