Job summary

Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) for the DeMay Living Center in Newark, NY. This 24‑hour per week evening shift role involves supervising CNAs, timely medication administration, and ensuring thorough documentation in a post‑acute/long‑term care setting.

The successful candidate will be a graduate of an accredited Practical Nursing program with NY licensure, BLS, and 1 year of experience preferred.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• 1 year experience preferred.
• Current licensure in the State of New York.
• BLS certification (AHA) is required.

Responsibilities

• Provide professional patient care to long‑term and post‑acute care residents.
• Direct and monitor care provided by CNAs; ensure documentation accuracy.
• Lead CNA team to complete quality, individualized personal care according to care plans.
• Communicate reports with physicians and nursing staff; transcribe orders as needed.
• Monitor resident status and report to charge nurse/supervisor promptly.
• Complete medication administration records and related documentation on time.
• Complete required MDS documentation and observations.
• Assist with direct ADL care as needed.

SUMMARY:

As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.

Key Responsibilities:

• Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
• Performs treatments and documents progress of same as per physician orders and facility protocol.
• Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.
• Directs CNA assigned to the team in the timely completion of quality individualized personal care in compliance with resident's individual care plans. Ensures CNA documentation is accurate and complete.
• Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
• Contacts physicians, transcribes orders and follows up with appropriate action to institute orders in timely manner.
• Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
• Completes pre-reviews of medication administration records as per facility protocols.
• Completes required MDS documentation and all related observations in a timely manner.
• Supports CNA team through assisting in direct ADL care of residents.

Minimum Qualifications:

• Graduate from an accredited School of Practical Nursing.
• 1 year experience preferred.

Required Licensure / Certification Skills:

• Current licensure in the State of New York.

PHYSICAL REQUIREMENTS:

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20-50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision‑making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask‑fit testing, and physical exams are common requirements.
