Legacy Health and Rehabilitation in Pleasant Grove, AL is hiring a Licensed Practical Nurse (LPN) to join our compassionate care team. You will administer medications, monitor residents, assist with ADLs, and collaborate with RNs and physicians to support rehabilitation goals in a long-term care setting.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with ADLs (bathing, dressing, feeding) as needed.
• Collaborate with rehabilitation team to support recovery goals (physical, occupational, speech therapy).
• Maintain accurate and timely documentation per facility policies and regulations.
• Provide emotional support and education to residents and families.

Position Summary: Licensed Practical Nurse ( LPN )

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. The LPN will provide direct nursing care to residents, assist with rehabilitation plans, and collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standards of care in our long-term care and rehabilitation facility.
