WILL COUNTY HEALTH DEPARTMENT & COMMUNITY HEALTH CENTER seeks a Licensed Practical Nurse (LPN) for CHC/015 to provide general nursing care and leadership under the supervision of the Primary Care Manager.

The role involves working with physicians and a multidisciplinary team across family, OB, Infectious Disease, and Behavioral Health areas, with occasional travel to satellite offices. Schedule is Monday-Thursday 9:00 am-5:00 pm and Friday 8:30 am-4:30 pm.

Job SummaryUnder the direct supervision of the Primary Care Manager, the applicant will provide general nursing care and leadership while working collaboratively with physicians and other multidisciplinary team members. Instructs individuals and families in health promotion and disease prevention; provides instruction in general health care and treatment to assigned clients in branches and clinics, working in concert with family physicians as necessary. The candidate will be expected to work in the clinic’s family, OB, Infectious Disease, and BH areas and travel to satellite offices. Schedule calls for Monday through Thursday, 9:00 am-5:00 pm, and Friday, 8:30 am-4:30 pm.
