Job summary

FutureCare Associates, Inc. is seeking an LPN in Baltimore, MD to provide hands-on resident care under RN supervision, assist in care planning and document changes.

Qualifications

• Must hold an active Maryland LPN license.
• 1 year of nursing experience preferred.
• Current CPR certification required.
• Demonstrates strong interpersonal communication and teamwork.
• Ability to document care and follow state/federal guidelines.

Responsibilities

• Under the direct supervision of the RN, assist in care planning and treatments for residents and consults with other nursing staff as appropriate.
• Demonstrate basic knowledge and skills required to care for the resident population on an assigned unit.
• Review resident medical records to ensure nursing practice is within State and Federal guidelines.
• Document care provided and report changes in resident status to shift supervisor.
• Review referrals for consultation, treatment or resolution of problems and ensure they align with documented nurse plans.
• Seek opportunities for professional growth and expansion of responsibilities to maintain competency.
• Set and implement goals and policies consistent with FutureCare’s philosophy of care.

Qualifications

• Must hold an active LPN license issued in the state of Maryland


• 1 year of nursing experience preferred


• Previous experience caring for residents in an acute or long-term health care facility a plus


• Team player with strong work ethic and a willingness to assist where needed


• Must possess a current CPR license


• Ability to demonstrate effective interpersonal communication with all personnel, residents, and families
