Responsibilities

• Provide nursing and supportive care according to the Interdisciplinary Plan of Care under the supervision of an RN.
• Communicate with physicians, team members, and other agencies to coordinate care.
• Document all services provided.
• Understand Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition to the RN.

Qualifications

• Current Florida LPN license
• Minimum 1 year of clinical experience as an LPN
• Current CPR certification
