The Pavilion at Jacksonville is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care and supervise CNAs in accordance with federal, state, and local regulations.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in the practicing state.
• Current CPR certification.
• IV certification, if applicable.

Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with regulations and policies.
• Promotes holistic care for residents using the nursing model.
• Ensures policy/compliance by nursing personnel assigned.
• Participates in admission, discharge, and transfers of residents.
• Assesses changes in residents' status and notifies physician and family.
• Transcribes physician orders to medical records and carries out orders.
• Prepares and administers medications as per orders and monitors effects.
• Performs wound care as ordered and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to the unit staff.
• Other duties as assigned.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents' status, notifying the physician and resident's family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians' orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians' orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable
