Job summary

Berkshire Center is a skilled nursing facility located in West Babylon, NY. We are seeking an experienced LPN to provide a high level of care on a part-time basis. This union position requires 3pm–7pm shifts five days per week, with rotating weekends.

Responsibilities include vital signs, medication administration, wound care, and patient support. NY licensed LPN required, with LTC experience preferred.

Qualifications

• Must meet shift requirements: 3pm start, 5 days per week, rotating weekends.
• LPN experience in nursing home or LTC required.
• Currently licensed by the State of New York (LPN).
• Attention to detail and observation skills to recognize condition changes.
• Ability to coordinate resident unit functions through team nursing assignments.
• Kind, helpful and compassionate.

• Communicate with residents’ family to provide advice and instructions.

Berkshire Center is a skilled nursing facility located in West Babylon, NY. Our mission is to provide the highest level of care for each of our residents. We are looking for an experienced LPN that can provide a high level of care. This is a union position.

JOB DUTIES:

The purpose of your job as a Licensed Practical Nurse is to perform a variety of care practices under the direction of the Registered Nurse and/or Supervisor of the unit/shift. This includes, but not limited to:

• Take and record measurements of blood pressure, temperature, heart rate etc.

• Observe residents under treatment to identify progress, side-effects of medications etc.

• Monitor residents’ condition including fluid intake and output and compose patient charts.

• Administer injections, prescribed medications, enemas etc., clean and dress wounds.

• Collect samples for lab testing as assigned.

• Provide emotional and psychological support when needed.

• Communicate with residents’ family or friends to provide advice, comfort, and release instructions.

QUALIFICATIONS:

• Please do not apply if you cannot meet the shift requirements (5 days per week and must be able to arrive at 3pm)

• Nursing home or LTC experience required

• Currently licensed by the State of New York..

• Possesses attention to detail and observation skills that enable the recognition of condition changes in the resident.

• Ability to coordinate all functions in the operation of a resident unit through effective team nursing assignments.
