As a Licensed Practical Nurse (LPN) on our day shift team, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our residents. About the Role:

• Provide direct patient care to residents, including administering medications, documenting medical records, and performing vital signs

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Monitor and report changes in patient condition to licensed nurses and physicians

• Participate in quality improvement initiatives to ensure exceptional patient care and outcomes

• Maintain a safe and clean environment for patients, staff, and visitors

• Document patient information accurately and promptly
