Trilogy Health Services, LLC is seeking an experienced Licensed Practical Nurse (LPN) for a hands-on role in our Lancaster, OH facility. The LPN will deliver direct clinical care, supervise nursing assistants, and ensure quality outcomes for residents on assigned units.

You will collaborate with a multidisciplinary team to monitor health status, respond to changes, and uphold safety standards. The position requires an active LPN license in Ohio and CPR certification.

Qualifications

• Active LPN license in Ohio (or eligible), CPR certification, and current healthcare credentials.
• Experience in long-term care or skilled nursing settings preferred.
• Strong communication and teamwork skills to coordinate with nursing staff and healthcare providers.

Responsibilities

• Visit residents daily to monitor physical and emotional status and report changes to charge nurse or physician.
• Identify problems and emergency situations and initiate lifesaving measures as needed.
• Provide holistic care within LPN scope and supervise nursing assistants on the unit.
• Monitor medications, supplies, and equipment to ensure adequate availability.
• Complete medical forms, charts, and relevant reports; maintain accurate documentation.
• Report occupational exposures per facility policies and procedures.

POSITION OVERVIEW

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required
