Wedgewood Rehabilitation and Nursing Center in New York is seeking a dedicated full-time evenings LPN who loves geriatrics and thrives in a family-like team setting. You will provide direct nursing care to residents and supervise CNAs to ensure smooth daily operations.

Sign-on bonus of $1,000 upon completion of probation. Responsibilities include medication administration, wound care, update of the 24-hour report, and accurate care documentation while maintaining resident safety and comfort.

Qualifications

• Must possess a current and active LPN license in NYS.
• Must be in good physical and mental health.
• Must be able to effectively communicate with others.
• Previous geriatric experience is preferred.

Responsibilities

• Maintain the safety of residents at all times.
• Administer medications as directed by physician orders.
• Change dressings and report wound changes to supervisor.
• Maintain the Daily 24-hour Report.
• Supervise CNAs in their duties.
• Assist CNAs with resident needs (toileting, call lights).
• Complete documentation for resident care.
• Perform other duties as assigned by supervisor.

The individual must have a love for Geriatrics and work comfortably within a team. Our facility is small and we take pride in our family like setting! Responsibilities include providing direct nursing care to resident and supervise the day-to-day to the nursing operations carried out by CNAs.

EDUCATION, EXPERIENCE AND QUALIFICATIONS:

• Must possess a current and active license to practice as an LPN in NYS.

DUTIES AND RESPONSIBILITIES:

• Maintain the safety to the residents at all times.
• Administer medications as directed by the physician orders.
• Change dressings as directed by the physician orders and report all changes in wounds to Nurse Manager and/or Designee.
• Maintain the Daily 24-hour Report.
• Supervise CNAs in the completion of their duties.
• Assist CNAs to help meet the needs of the residents when available (i.e. toileting, call lights, etc.)
• Complete documentation for resident care.
• Perform any other pertinent duties as assigned by the supervisor.
