Andrus on Hudson in Hastings-on-Hudson, NY is hiring Licensed Practical Nurses (LPNs) to join our compassionate nursing team. We offer FT and PT shifts with flexible schedules.

You will provide direct nursing care, administer medications, monitor residents, and collaborate with the interdisciplinary team to develop and implement care plans. Qualifications

• Current New York State LPN license
• Strong clinical, communication, and organizational skills
• Experience using PCC or another EMR
• Compassionate, resident-centered approach to care

• Dependable and committed to high-quality nursing care
• Experience in skilled nursing/long-term care preferred

Responsibilities

• Provide direct nursing care to residents according to care plans
• Administer medications and treatments per regulations and policies
• Monitor residents' conditions and report changes promptly
• Perform nursing assessments and contribute to care plans
• Communicate with residents, families, physicians, and care team
• Document nursing care, meds, treatments, and changes in condition
• Coordinate and maintain continuity of care

Andrus on Hudson is looking for compassionate and dedicated Licensed Practical Nurses (LPNs) to join our exceptional nursing team. What You'll Do

• Provide direct nursing care to residents in accordance with their individualized care plans
• Administer medications and treatments as ordered and in accordance with applicable regulations and facility policies
• Monitor residents’ conditions and promptly identify and report changes to the appropriate nursing staff
• Perform nursing assessments and contribute to the development and implementation of resident care plans
• Communicate effectively with residents, families, physicians, and members of the interdisciplinary care team
• Accurately document nursing care, medications, treatments, and changes in resident condition
• Assist with coordinating and maintaining continuity of care
• Provide guidance and support to Certified Nursing Assistants and other members of the care team
• Promote resident dignity, safety, comfort, and quality of life

What We're Looking For

• Current New York State Licensed Practical Nurse (LPN) license

• Prior experience using Point Click Care (PCC) or another electronic medical record software
• Compassionate, resident-centered approach to care

• Previous experience in skilled nursing, long-term care, rehabilitation, or assisted living preferred
