Fairport Rehab & Nursing Center seeks a compassionate Licensed Practical Nurse to join our geriatric care team. You will administer meds, assist with daily living, monitor residents, and contribute to individualized care plans with the interdisciplinary team.

Qualifications

• Must have an active LPN license and board certification where applicable.
• Experience delivering geriatric nursing care is preferred.

About the Role:

• Administering medications and treatments as ordered by physicians
• Assisting with personal care and hygiene needs
• Monitoring vital signs and reporting changes to healthcare team
• Participating in care planning and development of individualized care plans
• Providing emotional support and education to residents and their families
• Collaborating with other healthcare professionals to ensure seamless delivery of care

• Pay Range: $30.00 - $34.00 per hour with pay commensurate with certification and years of post-License experience
• Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts
• Available Shifts: Day (7:00am - 3:00pm), Evening (3:00pm - 11:00pm), Night (11:00pm-7:00AM)
• Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)
• Weekend Availability: Required
• Sign-On Bonus: $3000 Sign-On Bonus for Full-Time LPN's
