Job summary

CFG Health Systems, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care under RN/physician supervision. The role includes administering medications, monitoring conditions, and contributing to care plans in a safe, patient-centered environment.

The LPN will document in EHR/EMR, educate patients on treatments and self-care, and collaborate with the healthcare team. Evening shift delivers a $4.00 shift differential and requires compliance with HIPAA and safety standards.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current LPN license in the state of practice.
• Current CPR/BLS certification.
• Knowledge of nursing principles, practices, and procedures.

Responsibilities

• Deliver basic nursing care to patients per established policies and procedures.
• Monitor and record vital signs, symptoms and responses to treatment.
• Administer medications, injections and treatments within scope of practice.
• Assist RNs/physicians with patient assessments and care planning.
• Collect specimens and perform basic diagnostic tests.
• Maintain accurate, timely documentation in EHR/EMR.
• Educate patients on medications and self-care strategies.
• Observe and report changes in patient condition to supervisors.
• Maintain a safe, clean and therapeutic care environment.
• Collaborate with interdisciplinary healthcare team members.

LPN supports patient care activities, administers medications, monitors patient conditions, and assists with treatment plans while ensuring adherence to clinical protocols, safety standards, and regulatory requirements.
