Caretree Health Corporate is hiring a Licensed Practical Nurse (LPN) for all shifts in a busy Skilled Nursing Facility. Sign-on bonus available: FT $10,000; PT $5,000. Roles include administration of meds, patient care, and collaboration with physicians and RNs.

Qualifications

• Licensed Practical Nurse (LPN) license is required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health conditions, including vital signs and responses to treatments.
• Assist in developing and implementing individualized care plans.
• Provide nursing care for residents, including wound care and daily living activities.
• Collaborate with physicians, RNs, and other healthcare team members for coordinated care.
• Educate residents and families about health management and procedures.
• Ensure care complies with regulatory and facility guidelines.
• Respond to emergencies and provide critical care when necessary.
• Assist with training and supervising nursing assistants (CNA).

• Ability to provide direct resident care and provide clear instruction to care partners.
• Ability to listen to resident, family, or physician concerns and implement appropriate action.
