Job summary

Citadel Healthcare in Sterling, IL is seeking a Licensed Practical Nurse (LPN) for second or third shift to provide compassionate, resident-centered care under physician orders. A $2,500 sign-on bonus is offered for full-time LPNs joining the team.

You will assess and monitor residents, document care, coordinate with the IDT, and uphold infection control and HIPAA standards while supporting a safe, home-like environment.

Qualifications

• Nursing degree or diploma from an accredited college or university.
• Current license to practice as an LPN in the state.
• 1 year of experience preferred; long-term care experience preferred.
• Excellent written and verbal communication in English.
• Ability to adapt to a flexible schedule and occasional overnight travel.
• Ability to interact positively with residents, families, and staff.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with standards.
• Performs resident assessments, reviews care plans, and ensures appropriate care.
• Monitors vital signs and administers IV therapy within license limits; provides respiratory care.
• Documents nurse notes reflecting care and resident responses.
• Maintains safe environment and addresses safety risks as needed.
• Directs non-licensed staff to meet professional standards of care.
• Communicates information to residents, families, physicians, and facility staff as required.
• Ensures care plans are implemented and updated; recommends changes as needed.
• Adheres to infection control and HIPAA policies and procedures.
• Performs other duties as assigned.

Job description

We are offering a $2,500 sign-on bonus for full-time Licensed Practical Nurses joining our Second or Third Shift teams! The Citadel of Sterling is seeking a Licensed Practical Nurse (LPN) to join their team! As a Licensed Practical Nurse (LPN), you will be caring for a group of residents and assuming responsibility and accountability for them. Nursing is provided through assessment, implementation, and evaluation of the plan of care.

Licensed Practical Nurse (LPN) Duties and Responsibilities

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.
• Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments.
• Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.
• Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted.
• Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care.
• Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.
• Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards.
• Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance.
• Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance.
• All other duties, as assigned.

Licensed Practical (LPN) Nurse Qualifications and Skills

• Excellent written and verbal communication skills in English.
• Ability to multi-task and remain calm under stress.
• Analytical and problem-solving skills.
• Understands and follows infection control standards, policies and procedures.
• Must be able to adapt to a flexible work schedule and occasional overnight travel.
• Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.

Licensed Practical Nurse (LPN) Education and Experience

• Nursing Degree or diploma from an accredited College or University.
• Possess a current license to practice in the State as a Licensed Practical Nurse.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Physical Requirements

• Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
• Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
