Job summary

Signature HealthCARE of Roanoke Rapids seeks an experienced LPN/LVN to provide direct nursing care to residents, supervise nursing staff, and coordinate care plans within a team-based environment. The role emphasizes medication administration, wound care, ADLs support, and compliant documentation.

Ideal candidates hold an active LPN/LVN license in the state, with CPR/BLS preferred and at least 1 year of clinical nursing experience; long-term care and supervisory experience are a plus.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor residents’ physical and emotional status; notify the attending physician of any changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges per established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
