Job summary

Rosemont Care Center in Rosemont, PA, is seeking a Licensed Practical Nurse (LPN) to deliver compassionate care to residents and support families. This role involves writing care plans, administering medications and treatments, and supervising nursing assistants.

The 7am–3pm shift offers opportunities for advancement and meaningful bonds with residents. You will participate in rounds, review treatment sheets for accuracy, and coordinate admissions, discharges, and transfers.

Qualifications

• Current Licensed Practical Nursing (LPN) within the state of practice.
• One year of relevant nursing experience.
• Flexible schedule, including availability to work evenings, weekends and holidays as needed.
• CPR and first aid certifications are a plus.

Responsibilities

• Institutes appropriate nursing intervention to stabilize a resident's condition and/or prevent complications.
• Makes daily resident rounds to interview physical and emotional status and implement nursing interventions.
• Responds to resident and family concerns and ensures concerns are documented with resolutions communicated to staff.
• Coordinates admission, discharge and transfer of residents.
• Obtains report from the nurse being relieved and provides it to the incoming nurse; informs the charge nurse of status.

Responsibilities

• Institutes appropriate nursing intervention which might be required to stabilize a resident's condition and/or prevent complication.
• Makes daily resident rounds to interview physical and emotional status and to implement any required nursing interventions.
• Responds to resident and family concerns and ensures that each concern is documented and a resolution is initiated or communicated to appropriate staff.
• Coordinates admission, discharge and transfer of residents.
• Obtains report from nurse he or she is relieving, provides report to nurse coming on duty and keeps the charge nurse or others (as appropriate) informed about the status of residents and related matters.
• Prepares, administer and documents medications following accepted practices.
• Consults with licensed or registered nurse, physician or licensed independent practitioner.
• Clarifies any order or treatment regimen believed to be inaccurate, ineffective or contraindicated by consulting with the appropriate licensed practitioner.
• Other duties as assigned.
