Citadel Healthcare is seeking a Licensed Practical Nurse (LPN) to join our clinical team at The Citadel facility in Illinois. The LPN assumes responsibility for a group of residents during a shift and provides licensed care as ordered by physicians in compliance with state and federal regulations.

The role requires current Illinois LPN licensure and CPR certification, with preferred experience in long-term care.

Qualifications

• Must hold a current CPR certification.
• Must possess a current Illinois LPN license to practice.
• 1 year of experience preferred in nursing; long‑term care experience preferred.

Responsibilities

• Provides licensed nursing care to residents as ordered by physician and per applicable standards and regulations.
• Transcribes and communicates physician orders to care plans, MAR, and resident charts.
• Administers medications and treatments as ordered, and documents care provided.
• Monitors and records vital signs and patient status, including IV therapy within license limits and respiratory care as ordered.
• Maintains a clean, sanitary work area and ensures safety of residents; reports accidents or hazards.
• Directs day-to-day CNAs to ensure care is provided per standards.
• Completes admission, transfer, and discharge forms; updates care plans as needed.
• Generates nursing reports at shift start and end; communicates changes to oncoming staff.

Job description

The Citadel at Saint Anne Place is seeking a Licensed Practical Nurse (LPN) to join their team! The Licensed Practical Nurse (LPN) assumes responsibility and accountability for a group of residents for a shift of duty.

Licensed Practical Nurse (LPN) Duties and Responsibilities

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations


• Provides and receives nursing reports upon reporting in and ending shift duty hours.


• Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner.


• Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations.


• Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.


• Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments.


• Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required.


• Prepares and administers medications and performs treatments as ordered by the physician.


• Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.


• Completes the required forms upon admission, transfer, and/or discharge.


• Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted.


• Makes written and oral reports/recommendations concerning the activities of shift as required.


• Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards.


• All other duties, as assigned.

Licensed Practical Nurse (LPN) Skills and Abilities

• Excellent written and verbal communication skills in English.


• Ability to multi-task and remain calm under stress.


• Analytical and problem-solving skills.


• Understands and follows infection control standards, policies and procedures.


• Must be able to adapt to a flexible work schedule and occasional overnight travel.


• Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.

Licensed Practical Nurse (LPN) Education and Experience

• Current CPR Certification required.


• Possess a current license to practice in the State as a Licensed Practical Nurse.


• 1 year of experience preferred.


• Long-Term care experience preferred.

Physical Requirements

• Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required.


• Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
