Job summary

Aviata at The Palms in Palm Harbor, Florida is seeking a dedicated Licensed Practical Nurse (LPN) to join our health and rehabilitation team. You will deliver direct resident care under supervision, assist with daily living activities, and support individualized treatment plans.

We value compassion, reliability, and teamwork; CNAs are supervised, and a Falls/Pressure Sore program is in place to ensure quality care for all residents.

Qualifications

• Active LPN license in good standing.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Demonstrated compassion and dedication to resident care.

Responsibilities

• Assist in implementing individualized treatment plans.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore programs.
• Provide nursing services to residents.
• Evaluate residents' medical status on admission and during treatment.

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Aviata at The Palms in Palm Harbor, Florida. As an LPN at our health and rehabilitation center, you will provide direct patient care to our residents, assisting them in activities of daily living under the direction and supervision of a professional nurse.

Responsibilities:

• Assist in the implementation of individualized treatment plans for each assigned resident

• Supervise CNAs and participate in their evaluation

• Oversee Falls Program and Pressure Sore Program

• Provide nursing services

• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment

• Other duties as assigned

Qualifications:

• Must possess a current, unencumbered, active state license to practice as an LPN

• Experience in Skilled Nursing/Rehabilitation facilities preferred

• You must be qualified, compassionate, and dedicated to a job well done
