Job summary

Port Rehabilitation and Healthcare Center in Newburyport, Massachusetts is seeking a compassionate Licensed Practical Nurse (LPN) to join our skilled nursing and rehab team. Qualifications

• Current Massachusetts LPN license.
• High school diploma or equivalent.
• Strong communication and interpersonal skills.
• Compassionate, dependable, and committed to resident-centered care.
• Ability to work effectively as part of a multidisciplinary team.
• Long-term care, skilled nursing, or rehabilitation experience is a plus, but not required.

Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled nursing and rehabilitation setting.
• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents’ conditions and report changes promptly to the appropriate clinical team members.
• Perform vital signs, assessments, wound care, and other nursing duties within the LPN scope of practice.
• Assist with developing, implementing, and updating individualized care plans.
• Accurately document resident care, medications, treatments, and changes in condition.
• Communicate effectively with residents, families, physicians, RNs, CNAs, and other members of the interdisciplinary team.
• Support a safe, clean, and respectful environment for residents.
