Job summary

Care Options for Kids is seeking an LPN for pediatric home health in New Jersey. You will provide focused, one-on-one nursing care in the patient’s home, with RN oversight and clinical leadership support.

This role emphasizes meaningful patient relationships, clear direction, and safety, delivering care with confidence and consistency. Qualified candidates have a New Jersey LPN license, current BLS CPR, and experience with tracheostomy/ventilator-dependent patients; a willingness to train for

Qualifications

• Valid New Jersey LPN License or Multistate License
• Physical from within three years
• PPD or Chest X-Ray
• Valid BLS CPR card (obtained in person not online)
• Valid driver's license
• G-tube, trach, vent experience or willing to train

Responsibilities

• Provide one-on-one pediatric home health nursing care in the patient’s home under RN oversight
• Collaborate with RN and clinical leadership to implement care plans
• Maintain patient safety, documentation, and adherence to CDC guidelines
• Foster meaningful relationships with patients and families
