Job summary

Samaritan Daytop Village, Inc. is seeking a Licensed Practical Nurse (LPN) to support providers, record vital signs, administer methadone, and assist with procedures at our New York facilities.

Under the supervision of the RN and/or Physician, the LPN will assess, plan, and deliver nursing care, maintain documentation, and collaborate with the care team to meet participants' needs.

Qualifications

• High School Diploma or Equivalent.
• Graduate of an LPN program.
• NYS LPN license.
• BLS certification.
• Experience with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).
• Proficiency in Phlebotomy and EKG.
• Flexible schedule availability.
• Computer literacy including MS Office and EHR.

Responsibilities

• Assists with patient care including checking vital signs and blood draws as ordered.
• Observes, measures, records, and reports data related to patient status.
• Administers methadone under RN supervision.
• Prepares labels and take-home bottles of medication for self-administration.
• Counts and records medication at end of day; reports spills.
• Participates in ongoing assessment of patient needs.
• Follows methadone dispensing protocols per OASAS.
• Counts medications with RN during end-of-day closing.

The Role

Under the general supervision of the RN and/or Physician, the Licensed Practical Nurse-LPN will support providers in the provision of care including recording vital signs, administering methadone, discussing chief health concerns, performing testing, and assisting with procedures and documentation. Assesses, plans, evaluates, and delivers nursing care in collaboration with RN and/or Physician. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities

What You Will Do

• Assists with patient care including checking vital signs, blood draws, obtaining other specimens for laboratory analysis and culture as ordered by the medical provider, discussing chief health concerns, and administering injections and immunizations.
• Observes, measures, records, and reports data relating to patient mental and physical status.
• Administers methadone under the supervision of the RN.
• Prepares labels and take-home bottles of medication for self-administration by clients for periods when the medical clinic is closed or for clients who are permitted to self-medicate in accordance with agency policies.
• Records and reports any spillage that may occur while dispensing a patient’s medication.
• Participates in the ongoing assessment of patient’s needs.
• Follows protocols for methadone dispensing as established by OASAS.
• Tallies medication sheet at the end of the day.
• Under the supervision of an RN physically counts medication at the end of day closing.

Qualifications

Who You Will Be

• High School Diploma or Equivalent.
• Graduate of a formal Technical or Vocational LPN Program.
• NYS LPN license.
• BLS certification.
• At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI) or in a similar working environment.
• Proficiency in Phlebotomy and EKG.
• Available to work a flexible schedule as needed in response to participant and staff needs.
• Computer literacy including proficiency in Microsoft Office Suite and EHR.
