Summary

Provides basic nursing care by assisting in resident assessments on admission and ongoing. Communicates significant changes to the interdisciplinary team (IDT), documents assessments accurately, and participates in care planning. Implements nursing interventions, ensures safe medication administration, and updates care plans as needed. Administers IV fluids if certified, and uses critical thinking to monitor residents' conditions. Maintains professionalism during crises, and actively uses the QAPI process for problem resolution. Completes required continuing education, stays current with nursing practices, and may perform additional duties as assigned.

Job Qualifications
  • Graduated from an accredited nursing program resulting in LPN qualification
  • Current LPN license in the applicable state
  • CPR and AED certification, or willingness to obtain within six months
  • IV certification, or willingness to obtain within six months
  • Long-term care experience preferred
  • Ability to assess geriatric nursing care needs
  • Excellent communication skills
  • Strong time management and organizational skills
  • Physical ability to stand, walk, lift, and move residents and equipment safely
  • Manual dexterity and visual acuity
*CB Pay range: $26.96 - $43.25 per hour. Category: LPN.
