Job summary

Mt Holly Rehabilitation and Healthcare Center in New Jersey seeks a Licensed Practical Nurse (LPN) to join our compassionate care team. We offer 12-hour shifts and full-time, part-time, and Per-Diem opportunities with daily pay.

You will provide patient care per care plans, administer medications, monitor residents, and document accurately while collaborating with an interdisciplinary team to promote well-being and safe, person-centered care.

Qualifications

• Active LPN license in New Jersey (unencumbered).
• Current BLS (CPR) certification required.
• Ability to care for multiple residents in a fast-paced facility.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with accepted standards of nursing practice and the plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders and care plans.
• Support a safe, respectful, and therapeutic environment for residents and their families.
