Job summary

The Meadows of Leipsic, part of Trilogy Health Services, Leipsic, OH, is seeking an LPN to provide direct clinical care and supervise nursing assistants on assigned units.

You will observe and respond to residents' health changes, deliver holistic care within your scope, and work within a supportive, growth-oriented team environment.

Qualifications

• Must hold a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents daily to assess health status and notify nurse in charge of changes.
• Identify problems and emergency situations, initiating lifesaving measures if needed.
• Provide holistic care within the scope of the LPN license.
• Assist in inventory management of medications and supplies to ensure adequate stock.
• Complete medical forms, reports, evaluations, charting, and other documentation as necessary.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policies.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Qualifications
