North Bank Center for Rehabilitation and Healing in Jacksonville, FL is hiring Full-Time Licensed Practical Nurses (LPNs). Walk-In Interviews run Monday–Friday, 8:00 AM–5:00 PM at 333 E Ashley St, Jacksonville, FL 32202.

Join a diligent team providing inpatient care in a 120-bed facility. The role focuses on admissions, medication administration, infection control, and collaborating with an interdisciplinary team to ensure high-quality resident outcomes.

Formación

• LPN license required (state or multi-state).
• CPR certification required.
• Two years nursing experience preferred.

Responsabilidades

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control procedures and center policies.
• Collaborate with interdisciplinary team to optimize resident outcomes.
• Identify and report changes in residents’ condition per policy.
• Deliver nursing care for long-term or rehabilitative needs.
• Document patient/resident data and plan of care.
• Ensure compliance with state Nurse Practice Act and center procedures.

Educación

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
