Job summary
North Bank Center for Rehabilitation and Healing in Jacksonville, FL is hiring full-time Licensed Practical Nurses (LPNs) to join a 120-bed skilled nursing and rehabilitation center. You will provide nursing care, administer medications, and collaborate with an interdisciplinary team to promote residents' well-being.

Qualifications
- LPN license in good standing with state board.
- CPR certification required.
- Two years of prior nursing experience preferred.
Responsibilities
- Admissions, discharge and transfers of residents.
- Medication requisitions with pharmacy and physicians; medication administration.
- Follow infection control policies and center procedures.
- Collaborate with interdisciplinary team to achieve optimal resident outcomes.
- Identify and report changes in residents' condition.
- Provide nursing care under supervision per state nurse practice act and center policies.
- Document patient data and care observations; implement care plans.
- Direct and supervise other nursing personnel when applicable.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:
- Admissions, Discharge and Transfers of residents
- Medication requisitions with pharmacy and physicians
- Medication administration
- Following infection control, emergencies, and other center policies
- Collaborating with resident’s interdisciplinary team to provide the best outcomes
- Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
- Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
- Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
- Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
- According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
- In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
- Maintains knowledge of necessary documentation requirements.
- Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
- Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.
Licensed Practical Nurse (LPN) QUALIFICATIONS:
- LPN (Licensed Practical Nurse) licensed by the State or Multistate License
- CPR Certification required
- Two Years prior nursing experience preferred
