Trilogy Health Services, LLC seeks a dedicated Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants on their unit. The role emphasizes resident well-being, team collaboration, and adherence to care plans within a supportive environment.

The LPN will observe residents daily, respond to changes, and ensure timely reporting to the charge nurse and physicians. A CPR-certified LPN with an active license is required to join the team now.

Formación

• Current, valid state LPN license.
• Current, valid CPR certification.

Responsabilidades

• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify the charge nurse or physician when there is a change.
• Identify resident problems and emergency situations and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of the LPN license.
• Assist in monitoring medications, supplies and equipment to ensure adequate stock for residents.
• Perform administrative duties such as completing medical forms, reports and charting as necessary.
• Report occupational exposures to blood and infectious materials per facility policy.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required
